EVALUATION COST

The cost of an evaluation is variable and depends on the amount of time
dedicated to interviewing, testing, and report writing. An initial payment is due
at the time of testing, and the remainder of the balance can be paid over the
next 4 months. We are able to provide an estimated cost for the evaluation
based on the client's age, referral questions, and type of evaluation being
conducted. See "Neurofeedback and qEEG Brain Mapping" section of this
website for information about the cost of qEEG brain mapping and
neurofeedback sessions.

                         INSURANCE BILLING

Our office is not contracted with health insurance companies. Being contracted
with health insurance companies forces psychologists to shorten their
evaluations and feedback sessions, significantly reducing the
comprehensiveness of the evaluation. Also, neuropsychological evaluations
are typically only covered for neurological disorders such as head injury and
stroke, and learning disorders are never covered by health insurance. When
evaluating individuals for ADHD, autism-spectrum disorders, and other
psychiatric and developmental disorders, health insurance companies often
deny claims,
even with contracted/in-network providers. We are happy to
provide clients with superbills that can be submitted to insurance companies.
Here is the information to provide to your health insurance company to check
on what they are likely to cover if we evaluate you or your child:

Dr. Baker's NPI number:      1487868436

CPT codes typically used:   90791 (Diagnostic Interview, 1 hour)

PAYMENT

Payment can made in the form of check, cash, or credit card (Visa and
MasterCard). Checks should be made payable to “Baker Neuropsychology".

                 SCHEDULING/PREPARATION

All clients (or parents/guardians of children) are sent questionnaires prior to
the testing session. Parents/guardians are asked to fax, mail, or bring in the
questionnaires prior to the evaluation so we can review the materials and
potentially ask some additional questions before the testing session. For
children being evaluated, questionnaires are typically completed by teachers
or others familiar with the child’s behavior, and these are also typically sent
prior to the evaluation. For adults being seen for an evaluation, questionnaires
are typically sent as well. Whenever possible, it is helpful for adults to also
send questionnaires ahead of time.
