PAYMENT/COST
Insurance Billing
Dr. Baker is a contracted provider for Blue Cross Blue Shield (PPO), TriWest, and
Arizona Foundation for Medical Care. Neuropsychological evaluations are typically
covered by these insurance companies. However, claims are sometimes denied by
these insurance companies, and that decision is often based on the diagnosis (which
is difficult to know until testing has been completed). For accepted claims, the
amount owed is based on factors such as the deductible, copay, and coinsurance
amounts.
Child screenings and athlete screenings are typically not covered by insurance
companies.
Out-of-Pocket Costs
Child & Adult Neuropsychological Evaluations
The cost of a neuropsychological evaluation is variable and depends on the amount
of time dedicated to interviewing, testing, and report writing. For clients paying out
of pocket, an initial payment is due at the time of testing, and the remainder of the
balance can be paid over the next 4 months. For individuals with insurance plans
that Dr. Baker is not contracted with, a bill can be sent directly to the insurance
company or given to the client to submit. If the claim is accepted, Dr. Baker will
subtract the amount reimbursed from the amount still owed by the client. In certain
instances, Dr. Baker is able to conduct evaluations with non-contracted insurance
companies if the evaluation is pre-authorized ahead of time.
Neuropsychological Screenings
The cost of a screening is variable, depending on the depth of the screening, as well
as the length of the feedback session and report.
Athlete Neuropsychological Screening
The cost of an athlete screening is $200.
Payment
Payments should be made in the form of check, cash, or credit card (Visa and
MasterCard). Checks should be made payable to “Baker Neuropsychology and
Rehabilitation, PLLC". As previously noted, for full neuropsychological evaluations
paid out of pocket, an initial payment is due at the time of testing, and the remainder
of the balance can be paid over the next 4-6 months. Full payment is expected at
the time of service for screenings and treatment sessions.
SCHEDULING/PREPARATION
All clients (or parents/guardians of children) are sent questionnaires by e-mail and/or
mail prior to the testing session. Parents/guardians are asked to fax, mail, or bring in
the questionnaires prior to the evaluation so Dr. Baker can review the materials and
potentially ask some additional questions before the testing session. For children
being evaluated, questionnaires are typically completed by teachers or others
familiar with the child’s behavior, and these are also typically sent prior to the
evaluation. For adults being seen for an evaluation, questionnaires are typically sent
by e-mail. Whenever possible, it is helpful for adults to also send the questionnaire
ahead of time.
